I was gagged and muted for Nsfw and I don't know why, it also states I was warned and I do not recall such. I would appreciate if I was at least sent the video of what I said.

3.) Tell us why we should unban you:
I feel like I did nothing, and said nothing wrong, I know of nothing why I was muted or gagged other than allegedly I said something NSFW,

You told someone "I'm gonna touch you so hard" at the start of a round on the SCP jailbreak map, which is why you were muted. You had previously been warned to not say NSFW things, which is why the silence will be staying.
