Explain in detail what has happened & what lead you to your ban.
So the round ended and I was connecting to the new server when I was suddenly stopped saying I was banned. I had no idea why so I checked the bans tab and it said I was banned for "homophobic slurs in chat logs."
3.) Tell us why we should unban you:

Because I never changed my name in the last week or so, yet a guy named "Diamond Ingot" was banned, not me. This has to lead me to believe that there might of been an accident or something.

Hello.

The admin who banned you is offline right now, but I asked them about the ban. Please be patient until I can clear up what happened.

Never mind. I think there is sufficient proof to take action now. I found the player that was meant to be banned and replaced the ID on the ban. Ergo, you're unbanned.

However, I did find in your chat-logs remarks such as:

Firstly, if you have a complaint about a punishment, you should post it on the forums (like you did here) rather than call it "retarded." Secondly, insulting others is not allowed. I will not punish you for these remarks now, but take this as a warning that further offenses will result in a ban.
